Immunization Requirements for Kindergarten and 1st Grade
Children will not be enrolled in school unless an immunization record is presented and immunizations are up-to-date.
POLIO
- At least 3 doses
If last dose was given after the 4th birthday, one more dose is required. (A total of 4 is sufficient given anytime.)
DPT
- At least 4 doses
If last dose was given after the 4th birthday, one more dose is required. (A total of 5 is sufficient given anytime.)
MMR (Measles, Mumps & Rubella)
- 2 doses
Both doses must be given on or after the 1st birthday. Second dose can be given one to three months after 1st dose.
HEPATITIS B
- 3 doses
A three-dose series which takes 6 months to complete.
VARICELLA (Chicken Pox)
- 1 dose
Required for all pre-K, Kindergarten, and 1st Grade students. (Written documentation from a physician of having had the illness may be used instead.)
Physical Requirements for Kindergartners
A physical exam including TB risk assessment and anemia testing should be done no sooner than 6 months prior to entering kindergarten (March 1, 2011). Any physical exam completed before this date cannot be used.
Physical Requirements for First Graders
A physical exam including TB risk assessment and anemia testing after March 1, 2011, will be required for enrollment. MMR
- 2 doses

Tdap (Pertussis/Whooping Cough)
- 1 dose required, for 2011-12 school year, all students entering 7th through 12th grade
- for 2012-on, all incoming 7th graders need one dose Tdap
IMPORTANT NOTE: Your child will be required to have the Hepatitis B series (a three-shot series that takes 6 months to complete) and a 2nd dose of a measles-containing vaccine (usually given as MMR) before entering the 7th grade.

- Either 2-dose Formulation (Adult) or 3 dose Formulation
FDA approved 3-dose regimen vaccine manufactured by Merck & Co or SmithKline Beecham
or
2-dose regimen with the "adult" 10 mcg/dose vaccine manufactured by Merck & Co. (If certified by the health care provider that appropriate product was used for both doses)
